This treatment requires a patch test at least 24–48 hours before your appointment.
“I confirm that I have been advised to complete a patch test at least 24–48 hours before my treatment, but I have chosen to decline it. I understand that there is a risk of an allergic reaction, irritation, redness, swelling or sensitivity, even if I have previously received the same treatment without any problems. I confirm that I have disclosed all known allergies, medical conditions, medication, sensitivities and previous reactions. I understand and accept the additional risks of proceeding without a patch test and wish to continue with the treatment.”
